Supplemental Life Insurance
You can purchase additional life insurance for yourself, as well as coverage for your spouse/domestic partner and your child(ren). You pay the full cost of any supplemental life insurance coverage. You must purchase coverage for yourself in order to purchase it for your dependents.
Supplemental Employee Life Insurance:
- You may elect 1x – 10x salary in coverage to a maximum of $2,250,000.
- You will be required to show proof of good health if you elect coverage above $200,000 or if you decline coverage and decide to enroll later due to a qualified life event or future Annual Enrollment.
Spouse/Domestic Partner Life Insurance:
- You may purchase Life Insurance for your spouse or domestic partner in $10,000 increments, to the lesser of $250,000 or 100% of your employee supplemental life insurance amount.
- Guaranteed issue amount is $50,000; electing coverage above the guaranteed issue amount will require proof of good health.
Child Life Insurance:
- You may purchase life insurance in the amount of $10,000 for your child(ren).
